3. approval of guardian/s
4. yup
5. Attention. The signature has to be made into the orange box with the black pen in the presence of the passport official.
6. The passport official did something with them IIRC

Oh, didn't notice, the last one "may not be folded".

Signature
Passport applications must be signed in black ink in the orange box in the presence of the official receiving them. If mailed, the application must be signed in the presence of a notary public and be accompanied by an affidavit. The signature (place Paikka and date Päiväys) must be entered halfway down the form after the words Hakijan allekirjoitus.
