Im planning on requesting a passport application from the embassy in New York. is there anything that i need to include, like personal info self addressed stamped envelope? or just a little note?

(I think it's the Consulate General in NY, the Embassy's in Washington... but that's beside the point, you can apply from the NY Consulate anyway.)

My husband requested last month
And he just sent a letter asking for passport applications for himself and two minor children. We didn't include any identification or SASE.
The forms are in Finnish/Swedish of course but they do a little translating for you on a photocopied page (at least at Los Angeles Consulate) and between that and directions from web site the forms are easy to complete.
