Having trouble finding

I need a "certificate on employment pension insurance paid", as per the instructions in section E of OLE_TY forms(employment res-permit application).

I am with Elake tapiola.Everything returns to Finnish language.

I cant seem to find any phone number or request form that i can start the process of getting a certificate thats worthy of the res-permit extension.
Any hints/help is appreciated.
