Are you a back sleeper? Sounds like a chronic low-grade sinus infection or maybe it's the polyp itself that is bleeding and the blood is collecting while you do so, whereas normally you'd just be swallowing the slow trickle/ooze and it wouldn't be apparent. Of course only your doc can come up with a real answer. It's just oftimes hard to find the right doc.

This is all entertainment, of course, not actual diagnosis or advice. Consider it a football pool and when you do finally get it sorted through the HC system, we'll see who came closest.

As he persisted, I was obliged to tootle him gently at first and then, seeing no improvement, to trumpet him vigorously with my horn.

I thought about that, but I've had bleeding on and off for the past year and a half. I think I may have cracked it by saying that it's bleeding from my gums. I bought a very soft toothbrush and I'll be really careful brushing my teeth. I was just using a normal 'medium' one before. Occasionally I use(d) to get a tiny bit of bleeding from 2 or 3 of my top teeth after brushing. I was always careful and my dentist just said that I had sensitive gums and said to take a softer toothbrush and perhaps use sensitive-styled toothpaste.
Those electric brushes, ones which blink light if you brush too hard, are excellent. I'm astonished how easy I should go on my teeth with those.

edit. 1000 and higher models seem to have the pressure sensor: http://www.oralb.com/products/electric-toothbrush/

I've had occasional blood and mucus when I wake up, due to the dryness of the air, especially if it is the season where the leaves are popping out (see Vernal Catarrh, which is an allergy to the sap released when the leaves pop), or if it during the winter and all the windows are closed, the air is cold and bone-dry, and dust seeps in through those round vents that they have in the walls here. It helps some for us to do extra vacuuming then and to close the wall vents, and just open the windows from time to time for fresh air. My impression is that there is mold in the walls and that they don't line those air vents to the outside, so you can get mold coming into your flat through them as well as fresh air from outdoors.

Sometimes if a bed is really full of dust mites and their poop, a person can get bleeding in the nose from sleeping in it as well. Only solution there is to change the bed, I'm afraid, unless you have a cover on it you can wash on really hot. (Gross, but it has happened to us in the past)
