I applied for a permanent residence permit based on work.
Should my employer submit the "Terms of Employment" form mentioned in this link ?

Or does that apply only for the first extended work permit /type A) ?

Employer does not need to do that.
It is required when you're extending a work based residence, like EU Blue Card. That is why when you're applying to renew, it asks for employers contact details (assuming you're applying online), which it does not do when you're applying for PR.
